Download the User Transfer Form below. Print the form and add the information requested. Please be sure to Include the name and email address of the user to be transferred. Scan the form and email it to the Gallup Client Support Team (studentpoll@gallup.com), requesting the user account change. Gallup will transfer the account to the new school location within three working days and will send the requestor and email confirmation when it is completed.
